To claim your rebate, go online to rebate.monroe.com (the “Website”), or scan the QR code available at the participating Monroe distributors to take you to the Website, and follow the on-screen instructions to complete the rebate form with your first and last name, full address (No P.O. Boxes, except for residents of North Dakota), and valid email address for sending the Prepaid Card. You will be required to upload a copy of your original receipt (must be a pdf, .jpg or .png format and not exceed 8 MB) of a Qualifying Purchase and hit “Submit” to send your rebate submission. A QUALIFYING PURCHASE MUST BE COMPLETED BETWEEN APRIL 1, 2023 AND MAY 31, 2023 HOWEVER, THE WEBSITE WILL BE AVAILABLE FOR REBATE SUBMISSIONS UNTIL 11:59:59PM CT ON JUNE 30, 2023.
A Qualifying Purchase must be included on one (1) receipt. Your receipt must be an authentic retailer sales receipt, written in English, must be legible and include the following:
An email will be sent once your rebate form and receipt have been received, and another email after the sales receipt has been verified. Allow 48-72 hours for verification. Within sixty (60) days after submission verification, you will receive an email with a direct link to claim your Prepaid Card. You will have a choice at that time of activating your digital Prepaid Card for immediate use or requesting a plastic Prepaid Card to be mailed to you. Prepaid Cards will have an expiration date of six (6) months from the time of activation by the consumer. Prepaid Cards are subject to the restrictions, terms and conditions set by the issuer. Sponsor is not responsible for lost or stolen Prepaid Cards.
Information submitted in connection with this Offer will be subject to the Sponsor’s Privacy Statement at www.driv.com/privacy-policy.html. Sponsor is not responsible for network, hardware, software or server malfunctions, or other errors, failures, or delays in computer transmissions or network connections that are human or technical in nature that may affect your ability to participate or to access the website. All federal, state, and local income taxes associated with the receipt of the Prepaid Card are the sole responsibility of the recipients. All federal, state, and local laws and regulations apply.
